"CPR: Care Planning from Ratings"

When: 2pm - 3pm on Wednesday 2 September

During this webinar, the Queensland Mental Health Clinical Improvement Team will demonstrate how the use of outcome measures (specifically the HoNOS) can be used to assist in care planning. A vignette will be used to generate HoNOS ratings that, when combined with other relevant information, can form the basis of a clinically meaningful plan for the care of that consumer. The contribution of process elements, digital support and standards in documentation will be further outlined, with practical suggestions around the inclusion of outcome information into care planning requirements. It will also provide an insight into how these issues are being measured both quantitatively (KPI) and qualitatively (audit) in Queensland.​
